The ProMark Billing Process

We are here to help your patient throughout the ProMark billing process.

  • Step 1: If your patient chooses to determine his potential out-of-pocket costs before we run his ProMark test, please have him complete and sign the ProMark Patient Assistance Program Form.
    • This form provides us with the information we need to verify your patient’s insurance, determine his potential financial responsibility, and check his eligibility for the ProMark Patient Assistance Program.
    • If your patient does not complete this form, we will proceed with his test, and he will have an opportunity to apply for the ProMark Patient Assistance Program after his testing is complete.
  • Step 2: If your patient has completed the ProMark Patient Assistance Program Form at the time his ProMark test is ordered, we will review his insurance information and his eligibility for the ProMark Patient Assistance Program, and will contact your patient if his potential out-of-pocket  Costs for ProMark may exceed $350 to confirm whether he wants to proceed with the test.
  • Step 3: We will manage the ProMark claim submission process with your patient’s insurance plan. 
  • Step 4: Should there be a need to appeal your patient’s ProMark claim, we will handle this process with his insurer on his behalf whenever possible, and will support your patient in the process if he is required to submit an appeal directly.
  • Step 5: Once we receive your patient’s final EOB and any necessary appeals process has concluded, we will determine his responsibility for co-payment, co-insurance, or deductible, and will send your patient an invoice for the final amount due (if any).
    • We offer payment plans if your patient is unable to make a one-time payment.
