Your patient’s out-of-pocket costs
The ProMark billing process offers the opportunity for your patient to learn his potential out-of-pocket costs, if any, before we run his test. If your patient chooses to determine his out-of-pocket costs before we run his ProMark test, we will contact your patient if his costs may exceed $350 for confirmation that your patient would like to proceed with testing.

Your Metamark representative can answer any questions and provide additional copies of this form for your office.
